Your 3 Dental Plans
AMSURG offers 3 plans through Cigna. Your plan choices are:
- DHMO Plan: In-network benefits only; most services subject to a flat copay; includes coverage for orthodontia for children and adults.
- PPO Base Plan: Choice of in-network or out-of-network benefits; deductible and coinsurance apply.
- PPO Buy-Up Plan: Choice of in-network or out-of-network benefits; deductible and coinsurance apply; includes coverage for orthodontia for children and adults.
Find a Network Dentist
To confirm your dentist participates in the Cigna dental networks, contact Cigna at 1-800-244-6224 or take these steps:
- Go to www.cigna.com and click the Find a Doctor, Dentist or Facility button.
- When asked “How are you covered?”, click Employer or School.
- Enter an address, city or zip code, then click Doctor by Type.
- From the drop-down menu, select the type of provider you want (for example, Dentist or Child’s Dentist)
If you want to search for a DHMO dentist, select the Cigna Dental Care Access Plus network. If you want to search for a PPO dentist, search the Cigna Total DPPO network.

Things to Know
Dental ID Cards
If you enroll in a Cigna dental plan, you will not receive a physical ID card but can access your ID card electronically by downloading a digital copy at mycigna.com.
Pre-Determination of Benefits
If your dental service is expected to exceed $300, Cigna recommends getting a pre-determination of benefits. Cigna providers will coordinate this on your behalf.
